The short answer
Some insurance plans cover GLP-1 medications for weight loss. Many don't yet. Coverage depends on your specific plan, the medication (Wegovy vs Zepbound vs Ozempic vs Mounjaro), and what your prescriber writes the prescription for (weight management vs diabetes).
Wegovy and Zepbound are the "weight loss" brands
Wegovy (semaglutide, higher-dose formulation) and Zepbound (tirzepatide) are the two GLP-1 medications specifically FDA-approved for chronic weight management. When these are prescribed with a weight-loss diagnosis, some commercial insurance plans cover them — usually with a prior authorization step.
Increasingly common: plans cover Wegovy or Zepbound if your BMI is ≥30, or ≥27 with a weight-related condition. Some require you to try lifestyle interventions first.
Ozempic and Mounjaro are the "diabetes" brands
Ozempic (semaglutide) and Mounjaro (tirzepatide) are the same medications approved for type 2 diabetes. When prescribed for diabetes, they're covered by most plans. When prescribed off-label for weight loss, they're usually not — insurance sees the "diabetes-only" indication on your prescription and denies coverage.
"Ozempic" and "Wegovy" are the same molecule (semaglutide) at different doses and indications. "Mounjaro" and "Zepbound" are the same molecule (tirzepatide). Insurance treats them as separate drugs even though they're chemically identical.
Medicare and Medicaid
Traditional Medicare currently doesn't cover GLP-1 medications for weight loss (Congress excluded weight loss drugs from Part D in 2003 — that policy is under active debate but hasn't changed as of mid-2026). Medicare does cover Ozempic and Mounjaro for diabetes.
Medicaid coverage varies by state. Some states cover Wegovy or Zepbound with prior authorization; many don't.
Prior authorization: what it is, what to expect
Even when a plan covers GLP-1 medications, most require prior authorization — meaning your prescriber has to submit documentation showing you meet criteria before insurance will pay.
Typical prior auth requirements:
- Documented BMI meeting threshold (≥30 or ≥27 with comorbidities)
- Documentation of prior lifestyle intervention attempts
- Sometimes: confirmation you'll be under medical supervision during treatment

What if your insurance doesn't cover it?
You have three practical options:
- Manufacturer savings programs. Novo Nordisk (Wegovy, Ozempic) and Eli Lilly (Zepbound, Mounjaro) offer patient savings programs that can reduce out-of-pocket cost. Eligibility varies.
- Compounded semaglutide or tirzepatide. Compounding pharmacies produce these medications at significantly lower costs than brand-name products. This is what we prescribe when insurance doesn't cover the brand names. Same active molecule, dramatically lower cost.
- Cash pay for brand. Rarely the best option because the brand prices are high, but sometimes necessary.
